Take Your Medicine

I heard someone say, not long ago, that they had stopped taking a medication because it wasn't natural. I have been turning the word over ever since, because it is doing a great deal of quiet work and none of it holds up. Hemlock is natural. So are botulinum, ricin, and the cyanide in an apple seed. Insulin cultured in a steel tank is not. Whatever the word is tracking, it is not danger, and it is not the difference between what a body can use and what a laboratory made. What people seem to mean by natural is something more like old, familiar, and not obviously manufactured — and by that test, reaching for a drug may be the single oldest habit we have. Older, in fact, than being human.

Your dinner is already a pharmacy

Consider what you willingly put into yourself before breakfast is over. Caffeine is a nerve poison a plant evolved to kill the insects that chew it; we brew it deliberately and drink it for precisely the effect on the nervous system that the beetle was meant to suffer. Capsaicin is a deterrent the chili built to keep mammals off its fruit, which we have turned into cuisine on one hand and a topical painkiller on the other. Nutmeg in quantity is mildly psychoactive; the nightshades on your plate — potato, tomato, pepper — are cousins of tobacco and belladonna, carrying trace alkaloids of the same lineage. We do not choose foods only for their calories. We choose a good deal of what we eat for how its molecules rearrange us, and we always have.

Even the air qualifies. Oxygen is a corrosive, reactive gas; we survive it by running a slow, contained fire and carrying a full enzymatic staff to mop up the damage the fire does to us as we age. It is the most habitual drug there is — dosed every few seconds, fatal to overdose, fatal to withdraw — and we are so continuously on it that we mistake it for the baseline rather than the substance. The line between food, air, and drug is not a line of kind. It is a matter of dose and intent, which is the oldest lesson in pharmacology: the dose makes the poison, and the poison makes the cure.

Medicine is older than our species

The deliberate version — taking a substance as a medicine — shows up in every culture that left a record, using the same handful of tricks we still lean on. Willow bark for pain, its salicin the direct ancestor of the aspirin in your cabinet. Foxglove for the failing heart, which William Withering worked out in 1785 and which we still prescribe as digitalis. Poppy for agony. Cinchona bark for the fevers of malaria. The Egyptians wrote willow into the Ebers papyrus; the Sumerians pressed the poppy into clay. This is not the prehistory of medicine so much as medicine itself, wearing older clothes.

And it does not begin with us. Chimpanzees with gut parasites swallow whole the bristled leaves of Aspilia, not chewing, using the rough surface to scour worms from the intestinal wall — a dose with no nutritional point, timed to the infection. Capuchin monkeys crush millipedes and rub the toxic secretions through their fur as an insect repellent. Birds line their nests with medicinal plants; animals across the tree of life eat clay to bind the toxins in the rest of their diet. Biologists have a name for it, zoopharmacognosy, which is a heavy word for a simple fact: self-medication is not a human invention. It is an animal behavior we inherited, like grooming, and then refined.

Yeast made a bargain with the nervous ape

Alcohol is the deepest case of all, because here the drug and the organism that makes it reached into us at the level of the genome. Ripe fruit ferments where it falls; our ancestors ate it. About ten million years ago a single change to a single enzyme — a lone amino-acid swap in ADH4 — handed the common ancestor of humans, chimps, and gorillas a roughly fortyfold jump in the ability to metabolize ethanol. We were built to handle drink long before we were built to brew it. The scent of alcohol was the smell of ripe calories on the forest floor, and an appetite for it paid its own way. Robert Dudley calls the ancestor the drunken monkey, and the monkey is us.

Then, when we came down out of the trees and learned to ferment on purpose, alcohol did something no ripe fig ever could: it turned inhibition down on command. Consider what that unlocks in a creature like ours. A social ape can be too watchful, too self-conscious, too governed by the fear of the other's judgment to ever close the small distance between two people. Give that ape a reliable, temporary way to lower its own guard, and a great many meetings become possible that nerve alone would have prevented — and a great many of us are here, if we are honest about it, because our parents were briefly braver than sober judgment would have licensed. Whatever else that is, it is reproductive consequence, and reproductive consequence is the raw stuff evolution works on.

And the bargain runs both ways. A reversible off-switch does not only let the nervous ape occasionally be brave; it lets the whole lineage afford to be more careful than it otherwise could. Vigilance is expensive: the mind that scans every treeline and rehearses every slight is also the mind too clenched to sit easily among others or to court a mate. In an animal with no way to stand that guard down, selection has to keep the dial low enough that the watchman can still have a social life. Give the animal a switch that shuts the watchman off at dusk, reliably and temporarily, and the ceiling lifts — it can carry a more anxious, attentive, diligent baseline through the working day because it is no longer stuck with it through the night. Alcohol may thus have widened the band of temperaments a social species can hold at all: it bought us the disinhibition to reproduce and, on the very same account, the permission to be the rest of the time more watchful and wound-up than any animal without an off-switch could have gotten away with.

And this was no trace exposure. The dose, for most of history, was staggering. A sailor in the age of sail was entitled by regulation to a gallon of beer a day; when the beer spoiled on a long passage it gave way to a half-pint of rum, often over fifty per cent alcohol, which after Admiral Vernon's order of 1740 was cut with water and served out twice daily as grog — a spirit ration the Royal Navy did not finally abolish until 1970. Half an imperial pint of navy rum runs to eight or nine of today's standard drinks, issued every day, at sea, on watch. Ashore was much the same: medieval English laborers and monks drew allowances of roughly a gallon of ale a day, and by W. J. Rorabaugh's accounting the average American adult in 1830 drank about seven gallons of pure alcohol a year, some three times the rate now. Grant that a good deal of this was hydration in an age when the water could kill you, and that much of the beer was weak — discount hard for both, and the exposure is still daily, lifelong, and begun in childhood with small beer at the family table. We did not sip. For most of the human career we were, gently and continuously, at least half in drink.

So the honest way to read the relationship is not use but mutual domestication. Yeast manufactures a molecule that reaches straight into the primate reward system; the primate, thus captured, spends ten thousand years cultivating, selecting, breeding, and carrying that yeast to every habitable corner of the planet, until the little fungus that makes ethanol is among the most successful organisms alive. Some archaeologists think it was the wish to brew, not the wish to bake, that first bent us to the labor of farming grain — beer before bread. Richard Dawkins would call the ethanol an extended phenotype, the same way a flower's nectar is a bribe that conscripts a bee into unpaid courier work. We are, in part, yeast's bees. Humanity and yeast did not merely meet and trade. Each has been writing the other's evolutionary story for as long as there has been a story to write.

The pill is only the edit

If the habit is that old — older than farming, older than the species, spelled out in an enzyme — then the objection falls under its own weight. We have never once not been chemical animals reaching for other creatures' chemistry to steady, dull, sharpen, or unclench ourselves. What the last century actually changed is not the fact of medication but the craft of it. The willow gave you salicin and a bleeding stomach; we kept the analgesia and buffered the acid. The foxglove healed and killed by turns because the plant would not measure its own dose; the tablet measures it to the microgram. The old drugs arrived as blunt bundles — effect and side effect fused, take the whole thing or none of it. The new ones let us keep the effect we came for and file down the ones we didn't. That is not a departure from the natural history of medicine. It is that history, continued by more careful means.

Which leaves the person who sets the bottle down because it isn't natural holding the story precisely backwards. The molecule in the bottle is the same kind of thing our ancestors chewed bark and brewed fruit and swallowed rough leaves to get — minus the guesswork, and minus most of the harm. Taking it is not a break from nature. It may be the most faithfully human thing in the cabinet.
